By donating in honor of a care provider, grateful patients express their gratitude while honoring caregivers for all acts of compassion whether large, small, heroic, or humble.

Honor a compassionate caregiver with a donation to the Schwartz Center. The caregiver of your choice will receive a letter with your name, an explanation of the Honor Your Caregiver recognition program and a Compassionate Caregiver certificate. 

Make your gift online and select Tribute or send your donation by mail. Please be sure to include the name and mailing address of the recipient you are honoring.
