Criteria

• Located in the United States
• Be designated as a 501(c)(3) organization or have an identified fiduciary
• Demonstrate clear financial need
• Serve a population or patient mix including:

• • Historically marginalized populations, such as BIPOC and LGBTQ+
  • Geographically remote areas, where acute care facilities are limited
  • Medicaid-eligible patients and families

Application Process

• Participate in or view a recording of a Schwartz Center informational webinar prior to applying. Please submit our online inquiry form to receive upcoming dates and times and note ‘Stanzler Open Doors Fund’ in the additional comments section. 
• Have an identified project leader who will serve as the main point of contact with the Schwartz Center for the Stanzler Open Doors Fund application and materials.
• Have identified the Schwartz Rounds Leadership Team using our Roles and Responsibilities document to guide your selection process.
• Review, complete, and submit the Stanzler Open Doors Fund Readiness and Eligibility Form as part of the application.
• Provide a letter of support from a member of C-suite at their organization.

Program Requirements

• Commitment to host a minimum of two Schwartz Rounds sessions within the first year of implementation and a minimum of four Schwartz Rounds sessions per year thereafter for the duration of each funding cycle
• Agree to have a Schwartz Center staff person virtually observe and provide feedback for one Schwartz Rounds session within the first year of program implementation and every year thereafter
• Agree to collect and submit session and attendee summary data to the Schwartz Center after each Schwartz Rounds session
